Case management duties assisting clients with meeting needs in housing, employment, or other services as needed, creating service/treatment plans for clients to meet goals they set for recovery, and facilitating AM Psycho-educational Groups 1-2 times a week. Working with clients in treatment setting with clinicians and other staff/agencies involved for successful outcomes completing treatment program.
