This is achieved by one to one sessions, group work delivery and work with the family unit.- identify and develop strategies to support vulnerable and at risk young people with their behaviour, social and emotional development and help them overcome their barriers to participation in education or family life. - plan, develop, deliver, manage and maintain youth work programmes for young people during term time and school holidays.- ensure that young people who are disaffected and disadvantaged are engaged and their needs identified.- the delivery of safe, responsive and challenging youth work programmes to include opportunities for accredited and recorded learning, the promotion of cultural diversity, active citizenship, youth involvement and crime prevention.- ensure that the youth work programmes are delivered in a planned, systematic and coordinated manner.- liaise and work collaboratively across departments and with a range of agencies which include voluntary sector, schools, police and health authority.- To attend team around the family meetings, strategy meetings, child protection conferences and core group meetings.- make common assessment framework (CAF) referrals and child protection referrals if necessary to social care team. -
