Working in the primary care setting to provide chronic disease management and care for acute outpatient conditions.

One of two Nurse Practitioner fellows for the 2023-2024 Queen’s Medical Center’s Primary Care NP Fellowship Program. Assess, diagnose, treat, and manage acute and chronic conditions in the primary care setting. Rotated through specialties: pediatrics, urgent care, employee health, cardiology, geriatric house calls, outpatient neurology, pain and palliative care, inpatient psych, outpatient and inpatient diabetes, women’s health, and pre-surgery. Led an evidence-based project on remote blood pressure monitoring to better improve blood pressure outcomes within the primary care clinic. Used a multidisciplinary approach by working with clinic doctors, medical assistants, front desk staff, clinic coordinators, and managers. Resulted in over 70% of participants meeting their blood pressure goals within the 3 month time frame.

Oversees and directs care of 25 residents. Communicates with families and doctors regarding resident plan of care and changes in health status. Completes monthly summary reviews of residents. Trains newly hired nurses. Monitors and manages staff performance. Identifies need for in-service training and provides education to staff on a routine basis.

Provide care to children with diabetes – monitor patients’ blood glucose levels via CGM monitoring, complete routine insulin administration per MD orders, educate patients on carb counting, monitor for signs and symptoms of hyperglycemia/hypoglycemia. Provide care to G-tube dependent pediatric patients – initiate and monitor continuous enteral tube feeds per MD orders, monitor for aspiration/choking. Monitor seizure precaution patients and patients with migraine headaches, administer routine and PRN medications per MD orders.

Perform nutrition assessments for patients in ICU, telemetry, burn, oncology, med surg floors. Perform interventions and education for patients with nutrition-related conditions such as diabetes, weight management, cardiovascular disease, hypertension.

Graduate Assistant for the Undergraduate Research Opportunities Program (UROP). Promoted faculty-mentored undergraduate research and creative work opportunities to students via information sessions and orientations. Advised undergraduate students, faculty, and staff on UROP’s Project and Presentation funding opportunities, Summer Undergraduate Research Experience (SURE), and available opportunities on- and off-campus.

- Led presentations for 20-30 people/day on the prevention and reversal of heart disease through nutrition and lifestyle changes. Taught nutrition label reading, grocery shopping tips, dining out, portion sizes, and cooking techniques. - Developed Hawaii-inspired recipes for national Ornish website. Developed menu cycles, cookbooks, interactive cooking demonstrations, booklet with Ornish-approved products found in 4 common grocery stores on the island.- Developed a computerized Nutrition Tracker program to track participants’ weekly nutrition adherence and progress throughout the 9-week program and created the Ornish Dietitian Manual Handbook, including foodservice policies and procedures, menu cycles, food demonstration protocol, and program quality improvement goals.- Personalized diets for patients with additional chronic diseases, such as diabetes and chronic kidney disease.- Managed partnership with foodservice caterers to provide meals to participants and at Ornish-promotion events.- Promoted healthy eating and the Ornish program on local news station and to doctors, hospital administration, clinics, and potential participants at health fairs.- Multidisciplinary team approach leading to average participant weight loss of 11 lbs, 15.5% decrease in total cholesterol from baseline, 9.1% decrease in systolic blood pressure, 7.5% decrease in diastolic blood pressure within 9 weeks.
