Speech Language Pathologist
• Provide adult assessment, diagnosis, and treatment of speech-language, cognitive, voice, and swallowing disorders, from various etiologies.• Advise patients progressing from hospitals, SNF, and rehab facilities to home setting to increase safety and independence.• Attend and participate in weekly transdisciplinary team meetings, report patients’ current level of functioning, progress, and discuss discharge planning.• Communicate with PCPs to report safety issues, falls, medication discrepancies, request orders for important diagnostic assessments, OP referrals, and request specialist referrals (e.g., ENT, GI, or dietician services). • Request, track, and coordinate insurance authorization and justification for further services. • Oasis admissions, assessment, recertifications, and discharges. • Interpretation and application of oasis guidance as set forth by CMS.• Home safety assessments and creation of strategies and supports to increase safety, reduce falls, and reduce hospital re-admissions.• Collaboration with transdisciplinary team to carry out successful treatment plans to maximize Pt outcomes. • Care-partner education and training to increase carryover of HEP, safety skills, independence, and increase quality of life for patients.• Established new goal elements for the home health documentation software.• Created evidence-based HEP programs and educational resources used by SLP staff.• Produced home health processes document that was shared with new staff to decrease training time.Spearheaded the organization and administration of the offline SLP resources for the home health department.
