HoodenPyleGil (HPG) is a health systems research and innovation lab. My work with HPG was part of a $3 million grant to redesign how individuals with chronic illness receive care in New Haven, CT. ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~+ Developed strategy to identify critical stakeholders and align our project objective (reducing hospital and nursing home re-admission rates for patients with multiple chronic illnesses) with their business models resulting in the creation of four multidisciplinary working groups+ Designed and facilitated a project launch workshop for 40+ local healthcare, City and State leaders to reimagine a future state of health and happiness in New Haven, build project buy-in, and align all parties around a shared vision+ Deployed user-centric research methodology to inform project design; conducted 25+ interviews and focus groups to develop insights into the strengths and needs of patients and community stakeholder+ Evaluated and strengthened HPG’s human resources strategy to improve productivity and team dynamics by facilitating workshops and feedback sessions, resulting in a 23% increase in team productivity over a 2-month period
