Case management involves equipment prescription, home modification, service provision, liaison with service providers, education and training, hostel and nursing home placement.CPCT: Occupational Therapy services for clients with terminal cases living in the community. Involves equipment prescription, home modification and education, with a focus on client comfort and end-of-life issues. This includes pressure care management, energy conservation education and high level equipments needs such as hospital beds, electric recliners and alternating air mattresses.
