Coordinate, manage and analyse data collected across the state through the Home For Good Registry Week Campaign. Key tasks include:- Manage the data collection process through training volunteers and service providers and ensuring survey tool is fit for purpose- Analyse data collected in each region to develop a population profile of the housing, health and social needs of homeless people in the catchment- Present data profiles back to each region at local community briefings- Train and support service providers to access client information on a web-based database to facilitate effective case management

- Manage a specialist population health planning and performance team, including:- Manage and conduct external facing planning activities including health and service planning and whole of region needs assessments - Manage and implement internal planning and reporting processes including operational planning and a variety of reporting mechanisms- Manage and support business development processes including funding submissions and emerging partnership opportunities

Coordinate planning of all-of-region health and service plans for various identified priority areas. Responsibilities include:- Assessing service provision across the catchment- Conducting stakeholder consultation- Conducting consumer consultation- Ensuring alignment with current policy environment (national, state, local)- Conducting in-depth literature reviews to ensure evidence based practice

Key Tasks:- Advocate for preventative health interests to be represented in local governement planning- Provide advice/expertise to community organisations to support health promotion initiatives- Seek and support grants and funding for local government and community organisations.- Interpretate and translate research to inform health promotion planning, and practice- develop, implement, monitor and evaluate multi-strategy health promotion initiatives.- Provide accurate and timely health promotion advice and support to managers, partners and other stakeholders.

- Develop and implement reporting guidelines across all projects- Develop corporate fundraising strategies- Work at the grass-roots level of the orphanage, child sponsorship and community housing projects to recommend process improvement.- Liaise with and develop capacity in organisation staff to set aims, objectives and strategies for projects

Co-ordinator of various health promotion initiatives including- School based active transport, physical activity and nutrition programs- Corporate health programs (Health and Wellness)- Environmental health community education programs- Community based physical activity programs

Evaluate and propose evidence-based recommendations to improve a school based physical activity and nutrition initiative.
