Incoming PGY1 Internal medicine resident(Categorical).

Working in the ER, CCU, and the floors on a rotational basis during the pandemic was exhilarating. Took temperatures, case histories emphasizing on travel history performed physical examinations, EKG, assessed chest x-ray, and equipped me in handling emergency cases. During the surge in Bangalore, we saw a rush of patients with fluctuating saturation levels, some presenting with varied symptoms like diarrhoea, vomiting. I also got to work with the regular volume of patients with chest pain, abdominal pain cases with a perforated bowel. Understood how devastating the pandemic was, as patients either got better or worse. I felt more confident and comfortable with my work and learned about ventilators, and the adjustment for different patients depending on the vitals. Suggested the administration that we do a video call to counsel the family of the patients, which made a huge difference.

Obtained direct clinical experience working in an out-patient setting. Took histories and performed physical examinations, used the EMR-eclinical works with ease as I was comfortable from my prior clinical experience. Encountered patients with chronic disorders, obesity, cancers, CKD, autoimmune, infectious disorders and also got to see patients with rare disorders like Moyamoya disease, post heart transplant, cutaneous T cell lymphoma, Basal cell carcinoma of the skin and patients on watchman device. Performed out-patient procedures like otoscopy, COVID-19 antibody tests, suture removal, and wound care and also made presentations on the latest advances, which motivated me to keep myself updated. I mastered the art of history taking and the importance of effective patient care and witnessed the impact of the pandemic on people's mental health.

Granted a hands-on clerkship in internal medicine and cardiology. My responsibilities included obtaining case histories, performing physical examinations and EKG's, checking vitals: monitored stress tests, observed echocardiograms, and nuclear stress test. Documented the gathered information using e-clinical works for system-based practice and presented cases and treatment strategies for cases such as heart failure, atrial fibrillation, chronic disorders, cardiomyopathy, mental disorders, COPD, dysrhythmias, murmurs like aortic regurgitation, mitral stenosis, to name a few. We also had clinical discussions on the latest advances like inhaled insulin and the watchman device. In in-patient, I observed procedures like PCI, TEE. I was exposed to the difference in healthcare and adapted well to it and learned the impact of screening tests in preventing cancers. I improved my knowledge of reading EKG's and also was enlightened with various approaches to cardiac and non-cardiac related disorders.
