● Providing health promotion, disease and injury prevention, recommendations for clinical care, and support for older adults in the Rehabilitation Care Center and patients in Alternate Level of Care as well as Palliative Care patients within the Mackenzie Health Hospital ● Consulting with and/or referring clients to other health-care providers at any point in the care continuum when the client’s condition is not within the nurse practitioner’s scope of practice or the individual nurse practitioner’s competence.● Advocating for clients in relation to therapeutic interventions, health care access, the health care system and policy decisions that affect health and quality of life. ● Collaborating with members of the health care team to provide and promote interprofessional client-centered care at the individual, organizational and system levels
