Chronic Disease Prevention Program Specialist
Current•Promotes and maintains good working relationships with the public we serve, local healthcare agencies, healthcare providers internal and external and all aspect of duties are confidential •Facilitate and provides patient education on chronic obstructive pulmonary diseases and other medical complex diseases •Provides coordination and support in all aspects of patient education, and demonstrates ability to communicate in various ways and in appropriate formats to the target audience •Performs patients assessment and evaluation for determination of appropriate actions of disease intervention and care •Demonstrates respect and customer service to all of our patients, families, visitors, and fellow co-workers to ensure a professional, responsible, and courteous environment •Demonstrates knowledge and ability needed to perform complex medical procedures with minimal supervision •Provides update and patient status report on regular basis to department managers, doctors, nurses and pulmonary specialists in the outpatient clinics •Provides information, education, consultation for veterans with HIV/ AIDS and chronic pulmonary diseases who are required to perform pulmonary function tests to determine the severity of the diseases •Organizes and facilitates various conferences to promote public understanding of chronic diseases •Performs evident base-research on cureent practices, policies and recommendation at federal and state levels •Maintains accurate and legible records, documenting clinical notes in patients charts and ensure compliance with federal and states policies, The joint Commission (JCAHO), and meet regularly with the administrators from all levels •Maintains professional interactions with federal, other states, local agencies and the public with diverse background
