Hospital Pharmacist
CurrentI take the ownership of different Hospital Pharmacy processes. I review prescriptions for appropriateness, intervene when there is a medication error and recommend a suitable action to maintain care plan efficacy along with patient safety. I counsel patients compassionately about the effective and safe use of medications and ensure patient compliance . I am delegated as a quality coordinator for Pharmacy Department. I do Quality check on received medications from supplying stores and before distribution to other clinics. I control the safe handling and use of HIGH ALERT medications inside and outside pharmacy department. I apply clinical pharmacy and quality competencies to deploy risk management plan and enhance medication safety adopting system-thinking and multidisciplinary team approach. I use several quality tools like FOCUS PDCA, RCA and FMEA.I am a Cerner PharmNet super user, and I provide training and support for other colleagues.
