• Directs and manages the referral process for admission into the facility. • Evaluate patients' eligibility and complete psycho-social assessment considering utilization review and social worker’s need. • Obtains a nursing history and assess, diagnose, plan, implement, and evaluate in collaboration with the patient, family, physician, and multidisciplinary team, • Formulate nursing diagnoses based on the ICD code and create care plans based on assessment data. Ensure all services needed are identified. • Review and clinically support the continued stay of the patient as required by UM. • Assist UM to complete initial and concurrent reviews on open charts. • Consult with appropriate disciplines/departments as required to expedite care, obtain medical necessity documentation, and facilitate discharge. • Educates physicians and other staff on documentation to prevent denials in UM.
