Escort patients’ to exam room; collect, report, and document data, such as, patient’s chief complaint, head-to-toe assessment, vital signs, weight, height, head circumference, current medications, and oximetry level. Update the medical records for provider’s review. Perform lab tests, EKGs, spirometry, and document patient information in EHR.Perform patient education, pain management, and other interventions to restore/maintain patients' physical and psychosocial health. Maintain HIPPA guidelines.Call prescriptions into pharmacies; medication refills; document calls on prescriptions.Telephone triage; communicate/collaborate with health care team in patient care.Arrange consults, as well as outside labs and procedures.Provide age appropriate care and specific needs of the patients.Implement infection control techniques in all patient interactions.

Facilitate access, communication, and coordination to pediatric care needs, inclusive of referrals to specialist and subspecialist services that fosters family-centeredness, support for parental concerns, and promotes problem solving to encourage patients’ well-being.Provide continuous, culturally effective individual patient care coordination as appropriate and in collaboration with other health care professionals.Report to community health team (CHT) and meet with other care coordinators within the CHT network to support, train, and liaison with community resources, cross-pollinate ideas, and share experience.Collaborate with medical home staff to provide access to care on underinsured/uninsured patients; advise CHT manager on annual budgetary needs and responsible for appropriate documentation required for grant reimbursement of expenses. Make independent decisions regarding counseling, education, or referral of patients appropriate to scope of practice.Work with CHT leadership and staff, medical home leadership and staff, as well as community providers and non-profit organizations.

Serve as a resource to teachers and administrators in health education and as a member of the health curriculum committee.Coordinate the development of school policies related to health concerns.Prepare, implement, and evaluate individual student health care plans as needed.Provide protocols, training, supervision, and evaluation for delegated activities.Educate the school community about chronic and communicable diseases.Collaborate with local health care providers and local health agencies.Participate on the school crisis response team.Engage in evaluation of health services in collaboration with administration.Collaborate in promotion of staff wellness activities.Pursue professional development opportunities.Assess, plan, implement, and evaluate the health needs of students and the school environment.Provide health education information to students and staff.

Provide free health care services to adults as a registered nurse, as well as assist the outreach coordinator with clerical duties and reception of clients.
