The Irene V. Hylton Emergency Care Center at SNVMC has more than 40 exam rooms, including those for trauma, triage and minor injuries. Sentara's ER operates 24/7 and accepts patients of all acuity levels; SNVMC is a designated Primary Stroke Center. Documented patients of all ages, from minor injuries, mental health crises, labor and delivery, to trauma, including gun shot wounds, heart attacks, strokes, etc.Accompanied ER medical providers (Physicians, Physician Assistants, and Nurse Practitioners) into patient rooms to translate the patients' chief complaint and history into medical terms, then document the entire medical encounter into EPIC© electronic medical record system. Thus, helped reduce patient length of stay metrics by creating efficiencies for the medical providers, allowing them to solely focus on patient care. The medical record requires advanced medical terminology among various sections, including but not limited to the patient's medical, family, and social history; HPI/history of present illness; ROS/review of systems; physical exam findings; procedures; differential diagnosis; treatment plan; reassessments; physician consultations; EKG interpretations; vital signs; diagnostic study results; required documentation supporting a patient's code status (i.e. code stroke, code blue, etc); and discharge instructions. Trained new scribes on using EPIC and how to document various medical encounters. Also, I helped train new providers on utilizing EPIC©, including how to order diagnostic tests, input medications, write discharge instructions with any associated prescriptions, how to collaborate with and document medical specialists for appropriate patient follow-up care, and how to review patients' past medical encounters for appropriate health management, among others. In addition, I reviewed past medical records for accuracy and completion to maximize insurance reimbursements.
