Maintain client caseload of 32 highly vulnerable individuals, through a person-centered and harm reduction approach, by means of intensive case management, crisis intervention, informal counseling, and nonviolent de-escalation.Work directly with those who have experienced chronic homelessness, substance use, and neglected mental health, in addition to those who live with severe and persistent mental illness.Facilitate substance use and therapeutic groups aimed at whole-person wellness and stability.Promote success in maintaining housing and building a higher quality of life, by coordinating and collaborating with a wide variety of providers to advocate for client needs and benefits.Coordinate with local agencies and medical providers, to aid in the continuum of care.Provide outreach to future Housing First Tenants, and supports with their transition into housing.Promote and encourage the development of psychosocial skills, in addition to the building and improvement of activities of daily living.Assist with access and coordination of financial resources such as Social Security, Adult Public Assistance, Payees, Conservators/Guardians, and Alaska Native Corporations. Create and maintain Behavioral Health Treatment Plans, and proper therapeutic documentation through an electronic health care system.
