PRN at a skilled nursing facility

Family Health Center (July - September 2018)• Outpatient rehabilitation clinic – Neuromuscular and upper extremity conditions. • Diagnoses: Stroke, SCI, TBI, and basic upper extremity injuries • Age range: Adults• Assessment experience: Goniometry, MMT, Functional Capacity Assessment. • Treatment focus: Increase independence with ADL/IADL tasks by focusing on client’s abilities to match their goals from therapy. • Documentation experience: SOAP notes, evaluation notes, establish client’s goals and treatment plans. Alive! (September – December 2018)• Outpatient Hand therapy clinic – Upper extremity and orthopedic conditions. • Diagnoses: Tendonitis, fractures, edema, post-op, and other UE impairments. • Age range: Young adults • Assessment experience: 9-hole peg test, Monofilament, two-point discriminations, Goniometry, etc. • Treatment focus: Increase functional use of the upper extremity to increase independence with ADL/IADL activities. • Documentation experience: SOAP notes, evaluation notes, establish client’s goals, treatment plans and discharge notes.

SOAR (January – April 2017)• Community based clinic focuses on providing OT evaluation and treatment to elementary students who score significantly below benchmarks in literacy skills. • Diagnoses: ADHD, learning disabilities, and autism spectrum disorder. • Age range: Elementary school students • Assessment experience: Beery VMI, Sensory Profile, and Minnesota Handwriting Assessment.• Treatment focus: Increase literacy skills, behavioral skills, and social interaction. • Documentation experience: SOAP notes, evaluation notes, establish client’s goals and treatment plans. Work-to-Work (September - December 2017)• Outpatient program focuses on the development of higher-level independent living skills including work appropriate workplace communication/interpersonal skills. • Diagnoses: Autism spectrum disorder, learning disability, cognitive impairment and other mental health disorders. • Age range: Young adults • Assessment experience: Occupational Profile Interview, Worker Role Interview, Interest Checklist, Role Checklist, Occupation Self-Assessment (OSA), Independent Living Skills Test (ILS), Visual Screen, ROM Screen, MMT, Pinch and Grip strength, Beery VMI, COPM, Adult Sensory Profile, Assessment of Motor Process Skills (AMPS), Quick Neurological Screening Test-3, and Assessment of Communication & Interaction Skills (ACIS)• Treatment focus: Improve independent living skills and prevocational skills through participation in therapeutic social and tasks groups, one-on-one supervision during work activities and one-on-one sessions target specific skill areas for the client. • Documentation experience: SOAP notes, evaluation notes, establish client’s goals, treatment plans and discharge notes.
