Pharmacist
Current- Working with long-term care facilities to provide medication in the compliance dosage form.- Proficient with working on PACMED and PACVision machines for strip packaging.- Attending monthly meetings with doctors and nursing staff to discuss on Long term care patients' medication.- Weekly Long term care home visit to give the injection (Abilify, Risperdal, Testosterone).- Prescribing for minor ailments if needed.- Orders and communicates laboratory tests to the extent permitted by provincial regulations.- Level B and C compounding experience.- Collaborates with other health care professionals as necessary to identify and resolve drug therapy problems.- Adapts, extends, and modifies prescriptions as well as performs specific acts under the delegation or collaboration of a prescriber to the extent permitted by provincial regulation.- Works collaboratively with Registered technicians to ensure appropriate allocation of work tasks.- Effectively utilize Therapeutic knowledge to prepare care plans (CACP/SMMA) and communicate with other health care professionals when required.
