I am part of the glycemic management team at Salem Health. We see inpatients who are at risk for diabetes issues, including CBG and insulin issues, pumps, new to diabetes, new to insulin, new to diabets medications, gestational, and post cardiac surgery, etc. We also see patents in an outpatient setting and try to wrap around them to decrease readmissions.
