Supervise a team of about 10-12 Support Planners and oversee the services provided to Participants in the Community First Choice Program and Options Waiver. Assist in developing a person-centered plan with Participants to ensure their health and safety living in the community.

Manage direct support staff working in the homes with individuals with intellectual disabilities. Assist linking supported individuals to available community resources to help enhance their quality of life. Assist supported individuals with the development and implementation of their Individual Plan ensuring that the plan is person centered.

Worked in a direct care capacity provided case management services in a day program setting to adults with mental illness. Facilitated social and recreational activities as well as assisting and linking individuals to external resources in the community. Participated in the development and implementation on Individual Rehabilitation Plans and worked one on with with individuals by providing interventions to assist with achieving goals developed.

Provided case management services to adults with chronic mental illness is a residential setting. Assisted with activities of daily living, medication management, grocery shopping, doctor appointment, recreational and social activities.
