Oversees a staff of case managers and discharge care coordinators (85) responsible for patient care coordination. Develops and implements case management programs, including intake or discharge planning, and managed care contracting or negotiation. Evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Contributes to the development and improvement of clinical care pathways that enhance cost effectiveness while providing quality care.

Oversee day to day operations of medical clinic. Provide trauma focused cognitive behavioral therapy to children and adolescents. Weekly case staffing and collaborations with community partners.

Provide direct intervention with patients and families alongside of the health care team. Conduct assessments, provide problem solving, advocacy, community referrals and crisis intervention services ina multidisciplinary team environment. Accountable for the daily management of all complex discharge planning and social work functions for assigned patient care area using social work theory and clinical practice skills appropriate to the age of the patient while incorporating the patient or family’s social and emotional state.

Coordinates and completes initial and ongoing clinical comprehensive assessments; Consult with identified medical (primary care and psychiatric) and non-medical (DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process.

Provide Early Intervention services for children ages Birth-3. Works closely with the Early Intervention Service Coordinators relative to scheduling initial evaluations, follow up assessments, consultations and delivery of treatment services if not available through the CDSA provider network.

Provide clinical care in an 80 bed Inpatient Psychiatric setting through facilitation of group therapy, individual and family counseling, discharge and aftercare planning, utilization review and psycho-education. Develop discharge plan and assist with transition to Skilled Nursing Care, Assisted Living, or transition to home with home health services as decided by treatment team and family. Complete FL-2’s as needed for discharge plan.

Coordinates and oversees initial and ongoing assessment; develop the youth’s Person Centered Plan and ensure its implementation. Provide clinical expertise, guidance, and supervision to the IIH team members in the team’s interventions with the recipient. Consult with identified medical (primary care and psychiatric) and non-medical (DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process.

Facilitate daily program implementation in residential setting for 4 bed Level III mental health treatment center for females ages 11-17; Instrumental in new employee/intern development and training; Develop program and created manual for 6 bed DSS licensed facility for pregnant and parenting teenagers ages 12-23; Coordinating day to day tasks for clients to improve quality of life and behavioral functioning; Maintain Person Centered Plan objectives for each client while incorporating Cognitive Behavioral Therapy on a daily basis; Oversee/Managing 17 residential counselors while providing daily, weekly, and monthly supervision; Maintaining monthly and quarterly billing reports for agency Performance Quality Improvement process; Submit Inpatient Treatment Report for client authorization for care through North Carolina Medicaid and Health Choice and incident reports to Local Management Entity; Implemented Person Centered Plan objective for each client on a daily basis; Established and maintained professional working relationship with parents, schools, courts, social workers, case managers and all other parties involved in the treatment process.
