The Role: Own the on-the-ground COVID testing preparation and follow-through for our partnered assisted living facilities1. Ensure test days are conducted successfully and consistentlya. Ensure proper preparation for testing sites (patient registration, schedule encounters, prepare supplies)b. Ensure workflow checklists are completed fully and with high integrity c. Ensure reference materials are updated (EMR: Elation patient profile, facility master lists)2. Ensure patients are registered and their information documented properly (both new and returning)a. Ensure all registration information is inputted to the patient's chart and is accurateb. Ensure insurance information is inputted, or Billing Team has been notified for assistancec. Confirm information with the patient during check-in process ask for missing information3. Ensure Pine Park protocols are followeda. At the office: internal staff testing and office mask standards are upheld, standard workplace conductb. At the facility: safety and PPE standard are upheld, verbal confirmations of identity and insurance information4. Generally have an ownership mindset: your attention to both Elation and on-the-ground work directly impacts if and how patients are tested and receive results

1. Personal Care: Assist with daily personal hygiene tasks, including bathing, grooming, dressing, and toileting. Provide aid with mobility and transferring the residents from one place to another. 2. Meal Planning and Preparation: Planning nutritious meals according to dietary restrictions and preferences. As well as preparing meals, assisting with feeding if necessary, and help with maintaining proper hydration. 3. Household Tasks: Light housekeeping duties such as cleaning, laundry, and tidying up. 4. Companionship and Emotional Support: Provide companionship and engage in activities to promote social interaction and mental stimulation. Offer emotional support, actively listen, and engage in conversations to alleviate loneliness and promote well-being. 5. Monitoring and Reporting: Observe and report any changes in the person's condition or behavior to healthcare professionals or family members. Conduct check ins to track vital signs, maintain records, and document any concerns or incidents that occur during their shift. 6.
