Intensive Case Manager

• Coordinates with clinician, nurse practitioner, case manager, and director, comprehensive assessments; including but not limited to, clinical evaluations, psychiatric consultations, psychological testing, functional assessments, and social needs.• Develops collaborative case plans for each client, focusing on stable housing as a priority.• Monitors and follows up on case plan implementation through regular home visits, connecting clients with necessary services, and maintaining ongoing contact during treatment.• Manages community resources and involves clients to promote success with their case plan.• Interfaces with outside agencies for specialized services and care for clients’ success.• Assists clients in budget planning, fiscal resource management, and applying for entitlements.• Supports clients in finding suitable housing and adherence to United States Department of Housing and Urban Development (HUD) regulations.• Participates in multidisciplinary team meetings, in-service trainings, and required gatherings.• Maintains accurate documentation in required data systems, adhering to HUD regulations.• Shares confidential case information with supervisors and professionals, with client’s consent.• Generates timely activity reports and contributes to collecting clinical outcome data, as directed.• Facilitates access to community resources based on client needs, such as birth certificates, identification, social security, legal, and parole/probation issues, promoting social integration.As a bilingual interpreter, provides Haitian Creole oral interpretation for English as a second language clients, enabling them to comprehend complex program expectations and effectively communicate with the agency.
