• Knowledge of ProviderSoft and NYEIS• To orient the families during the initial process of an EI referral, evaluations and if eligible, the individualized family service plan meeting, ensuring that the families understands the process of Early Intervention.• To inform families that the program is voluntary, confidential, and at no cost to them• To discuss with families regarding the evaluation settings as well as discuss the rights to choose an evaluation site. • To notify families regarding eligibility requirements as well as provide timelines to complete the evaluations.• Explain to families regarding their family rights and their due process rights procedures. • To discuss child’s age out date and transition process.• To provide assistance in identifying the family’s needs for benefit and entitlement programs.• To maintain confidentiality of information for both clinical and office records.

• Manage client intake and assessment process to orient the client to the program and produce a comprehensive assessment of clients service needs into the AWARDS system.• Develop with the client a mutually agreed- upon individual service plan reflecting assessment finding goals to accomplish. Update the plan with the client as required.• Meet regularly with client on a weekly basis to assist with obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen clients ability to make appropriate life decisions.• Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.• Conduct weekly unit inspection.• Provide housing services such as: New York City Housing Authority ( NYCHA), CITY FEPS and LINC Vouchers• For individuals and families experiencing homelessness.

Supervise children between the ages of 2-13 years old, plan activities for children and create lesson plans.
