As a home care nurse, I see clients that need additional nursing support in their own homes as they recover from hospitalizations or transition back home from a nursing care facility. I interact with the client and their families, assess general health, care giver coping and report any abnormalities to the nursing case manager. Often, I act as a wellness coach, encouraging behavior that would improve or maintain my clients' health status.
