Project Director For The North Side CollaborativeThresholds Feb 2014 - Dec 2014Chicago, Illinois, UsThe North Side Collaborative is a pilot project funded by the Department of Mental Health in Illinois. In this project, Thresholds is working alongside Trilogy, C4, and Neumann Family Services to help prevent people with severe mental illness from unnecessary placement in hosptials or nursing homes. We offer crisis assessment, community support workers, temporary supportive housing, and other types of help to make a full life in the community a reality for people who would otherwise be institutionalized. My job is to help promote this service to hosptials and to help remove barriers to community integration.
