My current position as a clinical pharmacist allows me to work with a wonderful group of driven and passionate people. This environment grants me the opportunity to engage my talent, drive, and intellectual curiosity.I am responsible for yearly comprehensive medication review recommendations documented in the EMR and reviewed by primary care providers. In addition, I calculate renal function and assess current medications along with associated disease states to complete renal function review recommendations every 6 month for over 500 patients.I am lucky enough to serve as the primary pharmacy resource for a broad range of disciples, predominantly providing support for the physician and the nursing teams. I also provide facility wide medication education for the health care team and engage with patients to provide instruction and/or address concerns. I serve as a liaison between Innovage, participants, and the dispensing pharmacy. In addition, I work closely with NH and ALF staff to ensure appropriate medication prescribing and administration.Within the clinical pharmacist position I also have the opportunity to initiate and spearhead medication related projects affecting all Innovage locations within Colorado. I lead follow-up on medication error reporting and missing narcotic claims. As part of the Falls Team Committee I review all participant falls for contributing medication therapies and engage with multiple disciples (PT, OT, NP, Dietary) to formulate a plan for prevention of future events.
