• Provides screening, assessment, individual counseling, and group counseling.• Conducts assessments and performs therapeutic interventions in an outpatient setting.• Makes referrals to appropriate follow-up services, including medication-assisted treatment.• Documents all patient interactions with DSHS/CARF standards.• Develops SUD treatment plan in conjunction with patient, family/LAR and MH team members. • Ensures all required paperwork is completed and signed by all team members and meets DSHS standards. • Provides on-going case coordination services to patients including documentation of patient follow-up and staff interaction with other service providers (i.e., parole, probation, and other substance Programs). • Develops discharge plans and summaries per DSHS/CARF standards and refers patients to other Community resources as clinically indicated.• Perform other job duties or responsibilities as requested or assigned.
