Rotational Pharmacist
CurrentUndertaken various rotations in health services for elderly people (HSEP), surgery, stroke, cardiology wards and acute admission unit (AAU). Clinical review of patients and liaise closely with ward team during the rotations. Joined board and ward rounds with different members of ward team including the consultants (improving prescribing practice and minimising prescribing errors and medication-related harms), liaised closely with specialised teams such as pain team, micro consultants, diabetes team, anticoagulation team and palliative care team (which involves ward reviews of patients with the teams in order to optimise patient care).Joined a stroke MDT meeting with PT/OT, social workers, SALT, nurses, doctors and consultant in optimising patient care and facilitating discharge.Undertaken a clinical audit on the judicious use of proton pump inhibitors in AAU. Audit poster in AAU doctor’s office.Invited by a medical registrar to participate in the quality improvement project on polypharmacy in elderly patients (using Beer’s criteria) in the health services for elderly people (HSEP) ward.Experience in managing inpatient and outpatient pharmacy clinical queries.Ward rounds with acute medical, stroke, cardiology and geriatric consultants.Shadowed learning disability, heart failure, anticoagulation, diabetes, palliative, pain clinical specialist nurses to understand their role in the multidisciplinary team and how they review/manage patients.Involved in the write-up of local trust protocol on the use of IV methylpred for visually threatening GCA under the supervision of a formulary pharmacist with the rheumatology registrar, Acute medical consultant and rheumatologist.3 monthly Medicines management and CD audits in wards.Mentor to a trainee pharmacist. Training/teaching junior pharmacists and pharmacy students.
