I am responsible for creating and maintaining the CarePartners program in York County. I currently supervise staff members and am responsible for planning, developing, organizing, conducting, and evaluating, all aspects of the program to improve public health. Actively engaged and am responsible for maintaining strict records for the three grants that I either manage or help manage. I work in close relationship to many foundations, clinics, programs, and hospitals throughout Maine to keep a working relationship to maximize effectiveness. I also continue to provide prescription assistance to those within the program and am a liaison for the MedAccess program as I have the ability to interpret pharmaceutical terminology including a wide variety of chemical compound names, quantification of medication needs based on prescribed dose, frequency, available dosage forms, respond to minor inquiries regarding medication distribution from other healthcare professionals. This position requires a complete understanding of the controlled substance program. I also assist with case management with coworkers by developing appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients. I provide on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the clients to achieve the stated goals and objectives. I also have face to face reviews with the client and/or relevant staff on a regular basis to make progress in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved.
