    SOCIAL WORKER INTAKE COORDINATOR: The Social Worker Intake Coordinator functions under the Behavioral Health Care Line. The incumbent functions as a clinical lead of the Screening Team for Substance Abuse Residential Rehabilitation Treatment Program (SARRTP) and has significant responsibility for coordinating the referrals to SARRTP. The incumbent has the primary responsibility to coordinate the screening of veterans referred for admission to the program. The Intake Coordinator works closely with all referral sources such as the Inpatient Psychiatric Unit, Providers of Detoxification Services, Health Care for Homeless Veterans (HCHV), the Behavioral Health Outpatient Clinic, GEC, Care Coordination Team, other VA facilities and, community agencies. The Coordinator arranges referral or services on behalf of the veteran to assure that necessary and appropriate clinical services are received. The Intake Coordinator coordinates day-to-day functions of the admissions program that treats a variety of clinically challenging patients requiring residential placement into programming that provides support and services for homelessness, substance abuse, mental health, vocational, and related psychiatric and medical conditions. The SARRTP Admissions Coordinator may also assist SARRTP staff with discharge planning for those veterans no longer needing residential support as they return to independent functioning in the community, or to a higher level of residential support as warranted by the individual recovery plan. The SARRTP Admissions Coordinator supports Substance Use Disorder Treatment Program outpatient intake evaluation.
