- Manage a team of ten housing counselors; Each counselor holds a caseload of 12 to 25 clients; Conduct regular supervision with staff; Ensure all staff are in compliance with the NYS Office of Mental Health, Department of Health, and Medicaid Redesigned contracts documentation on all case file.- Assist adults living with severe and persistent mental illness with a multitude of dimensions including but not limited to; adult daily living skills, community habituation, social skills training, person centered treatment plans, medication compliance, scheduling and maintaining appointments (Therapeutic, Psychiatric, and Medical), roommate relationship mediation, budgeting, meal planning and prepping, moving, achieving longer periods of sustained sobriety through encouraging connections to MICA, AA, and NA support programs; Ensure treatment requirements are met for individuals with forensic histories- Maintain and routinely internally audit over 95 supported housing apartments on Staten Island to make certain apartments meet safety standards outlined by the NYS Office of Mental Health and the Department of Health regulations, track and see through all apartment issues until they reach resolution.- Helped staff coordinate ongoing vital individual service plans as well as care coordination for individuals on their caseloads as clientele reintegrated back into the community using proven data driven methods.- Collected and analyzed statistical data on program outcomes to integrate in grant submissions to New York state securing more funding to help more adults with mental illness have the opportunity to reintegrate back into the community.
