Next-Generation Sequencing (NGS): Caris has one of the most advanced sequencing laboratories in the world, enabling Whole Exome (DNA) and Whole Transcriptome (RNA) sequencing on every patient.DNA Sequencing: Whole Exome DNA sequencing is performed on all tumor types and for every patient, enabling accurate detection of mutations, copy number variations, insertions and deletions in a large number of cancer-relevant genes, plus precisely characterize homologous recombination deficiency* (HRD), genome-wide loss of heterozygosity (LOH), microsatellite instability (MSI), tumor mutational burden (TMB) and human leukocyte antigen (HLA) genotyping*.RNA Sequencing: Whole Transcriptome RNA Sequencing analysis is performed on every patient and is the superior method to detect clinically relevant aberrations such as gene-fusion events and splice variants. RNA provides sequence information after gene splicing has occurred, thus avoiding the complexities of genomic features of DNA, such as large intronic regions. Caris molecular profiling has the ability to detect rare fusion events far better than DNA-based short-read methods, and provides broad coverage of all exons, capturing all possible fusion partners.Immunohistochemistry (IHC): Caris evaluates protein biomarkers through an extensive immunohistochemical panel analyzed in a tumor-type specific manner to conserve tissue and avoid unnecessary testing. Our large team of board-certified expert pathologists utilizes digital pathology, overseeing application and interpretation of this technology.In situ Hybridization (ISH): For select genes, we offer ISH to provide higher level of resolution to our IHC results to detect amplification.Pyrosequencing (PyroSeq): We offer methylation analysis of the MGMT promoter to guide therapy selection in glioma.

The Ivy Brain Tumor Center is home to the largest Phase 0 clinical trials program in the world and the first of its kind for neuro-oncology. We have adapted the Phase 0 strategy to brain tumor patients and incorporated a clinical trial ‘trigger’ that advances Phase 0 patients into a therapeutic study arm only if the drug is having an effect in an individual patient.This approach addresses the two most common reasons most drugs fail in brain tumor patients:Most new drugs are incapable of reaching the brain.Laboratory animal studies of new drugs paint an incomplete picture of how the drugs will perform in humans.In a conventional Phase 1, 2, or 3 clinical trials, a patient may be on an experimental drug for months before learning that the therapy is ineffective. At the Ivy Center, we understand that time is the most important commodity for our patients. Therefore, our Phase 0 clinical trials match patients to first-in-class drug combinations, confirm drug effects within days of exposure, and only treat patients when therapies are active in a patient’s own tumor.

The goal of our research is to identify novel pharmacological and behavioral interventions for the treatment of drug abuse, and to explore the neurobiological substrates of addiction. Our work has revealed novel neurobiological mechanisms of nicotine addiction, and has the potential to contribute to the development of novel therapeutic options aimed at reversing nicotine-induced alterations and thus improve smoking cessation outcomes. This work has resulted in translational collaborations to examine clinical efficacy of pharmacotherapeutics in promoting smoking cessation.The lab examines the neurobiological underpinnings of cue-triggered motivation and conditioned reinforcement, specifically examining alterations in synaptic plasticity via dendritic spine morphology and glutamatergic signaling in nicotine self-administration and reinstatement. This research utilizes both in vivoand in vitro methodologies to study rapid alterations in synaptic plasticity (measured as changes in dendritic spines or AMPA/NMDA ratios) during or immediately following behavior (specifically, during reinstatement of cocaine or nicotine seeking).
