- Initial clinical staff member for Network 180’s new Behavioral Health Home (opened September 2016); an outpatient integrated behavioral health, physical health, and substance use disorder clinic with the focus on reducing the utilization of E.D. visits and inpatient hospitalizations through the support of a multidisciplinary team under a targeted case management authorization.- Operate within a trauma informed environment by participating in trauma informed care (TIC) committee to identify and implement agency and systemic areas of improvement regarding client care, agency environment, and vicarious trauma. -Function collaboratively through shared mutual cases with internal case management, peer support, nursing, psychiatry, and supervisory staff, while frequently coordinating with Network 180 Access Center regarding crisis. -Maintain continuous, long term, rapport with culturally diverse individuals presenting with a range of high acuity behaviors, severe mental health diagnosis, physical health struggles, historical compound trauma, familial/domestic abuse, parole/probate involvement,and substance use disorders, followed by an array of biopsychosocialspiritual needs. -Maintain a full caseload and demonstrate self-efficacy with the completion of intakes, biopsychosocial assessments, IPOS pre-plans, IPOS, IPOS quarterly reviews, addendums, service notes, and discharges.-Utilize an array of measurement tools including GAD7, PHQ9, and LOCUS to determine needs, level of activation, stage of change, and progress over time.-Institute clinical judgment when formulating DSM V diagnoses.-Conduct weekly individual therapy sessions with the use of exclusively evidence-based practices; ranging from DBT, CBT, ACT, Seeking Safety, ASIST, AMSR, Motivational Interviewing, EMDR, and Art Therapy.-Provide additional crisis support through a weekly in-house crisis telephone rotation.

- Fulfill a diversity of roles ranging from in-home clinical therapy to family case management. -Maintain a full case load while adhering to program documentation, production, hourly, tracking, on-call, travel, and client care expectations in a concise, organized, timely, fashion. -Coordination of care and advocacy with a diversity of community service providers including schools, primary care physicians, psychiatrists, out-patient therapists, in-patient psychiatric hospitals, emergency departments, probation officers, judges, juvenile detention, foster care workers, wrap around coordinators, parent support partners, residential placements, insurance providers, and Network 180.-Weekly tracking of Network 180 provided units/re-authorizations, production, progress notes, mileage/scheduling, referrals, bio-psycho-social clinical assessments, initial/quarterly/annual treatment plans, and discharge documentation-Utilization of CAFAS clinical measurement tool to monitor client progress.-Institute clinical judgment when formulating DSM V diagnoses.-Engage in crisis on-call rotation; coaching families through crisis calls, providing crisis assessments at Network 180/Emergency Departments, and using clinical judgment to determine appropriate level of care based off presenting symptoms and available resources.-Maintain continuous rapport with culturally diverse families presenting with high acuity behaviors, physical health struggles, historical compound trauma, and substance use disorders, followed by an array of familial needs.-Frequent use of CBT, DBT/Mindfulness, MI, ACT, WRAP, and Art Therapy modalities in sessions weekly.-Obtain weekly supervision from LMSW team lead.-Participate in regularly provided professional development trainings.
