The Investigations Department’s overarching objective is to make the industry safer. We investigate casualties to understand what, why and how a casualty occurred in order to identify lessons to be learned and changes that can be made to improve safety at the highest levels. We do not look for someone to blame or apportion liability. Fair treatment of seafarers is a core principle of how we work. As well as heading up a small team of skilled investigators, my role requires engagement at international level - attending the International Maritime Organization as part of The Bahamas delegation to provide insight gleaned for casualty investigation to working groups and involvement with Marine Accident Investigators’ International Forum (MAIIF) - an international non-profit organisation dedicated to the advancement of maritime safety and the prevention of marine pollution through the exchange of ideas, experiences and information acquired in marine accident investigation.

Marine surveyor with a particular focus on incident investigation and analysis - as well as being a member of BMT’s Collision Reconstruction and Simulation Team, I have investigated a number of high profile accidents from a flag state, coastal state and P&I perspective, with incidents ranging from certification fraud to cruise ship groundings, lifting equipment failure, fires, explosions, sinkings and personal injuries. UK lead for offshore renewables - providing project planning advice, program management, marine warranty services and due diligence surveys on windfarms and tidal projects in the North Sea and beyond.

Specialist investigator in the compliance group of Maritime New Zealand with delegations and warrants under the Maritime Transport Act 1994 and the Health and Safety at Work Act 2015.In this incredibly varied role, I investigated incidents and accidents including certification fraud, cruise ship groundings, lifting equipment failure, fires, sinkings and pressure vessel failure. Outcomes of investigations include safety bulletins, rule review, field staff training, safety campaigns and prosecutions under both Acts.As well as expanding my knowledge of the regulatory sector, in this role I expanded my investigation skills with training and development in scene examination, chain of custody, PEACE interview techniques and fire investigation (with Fire & Emergency NZ).
