Served as an advocate for 35 individuals comprised of children, young adults, adults and elders with varying intellectual impairments. Assist the individuals and their families to access services / resources in the community and within the school system. Assist with coordination of services (doctors, psychologists, specialists) and monitor and adjust those services based on individuals needs. Apply for and maintain federal and state funded benefits. Ensure that each individual receives the necessary services to maintain quality of life with the utmost independence. Maintain on-going documentation and case notes for monthly Medicaid billing in accordance with the agency policies and state regulations. Served as a Medicaid Service Coordinator Supervisor for 2 years, supervising 6 MSC's on a bi-weekly basis individually and in group.
