Intensive Case Managers and ACT Team members send me weekly reports of how patients whom have AOT court orders are functioning. AOT's whole purpose is to ensure that chronically mentally ill patients get the treatment that they need to function safely in the community. I help ensure that the patients get the treatment (psychiatric, psychological, social work therapy, individual therapy, group therapy, family therapy, chemical dependency/rehab, medical, concrete services, housing, medical, legal assistance, and advocacy) that they need via Intensive Case Managers and ACT Team members. It is such a FUN career!• Review and evaluation of documentation that is submitted by Care Managers (Intensive Case Managers, Supportive Case Managers, and ACT Team members) on the client's progress and participation in treatment• Responsible for monthly verification that all services on an AOT consumer's treatment plan are being delivered• Evaluation of paperwork to see if a Removal Request will be initiated for an AOT consumer that does not comply with their treatment and is at risk of psychiatric decompensation (the following criteria must be met: refusal or failure to comply with the court ordered treatment plan; evidence of efforts to solicit compliance; patient has been deemed missing for at least 48 hours ; clinical judgment that the consumer may be in need of an involuntary admission to a hospital)• Immediate cancelation of Removal, if the client has been located in a jail or hospital• Assisting Care Managers with effective problem solving (via telephone contacts and case conferences) with various issues that impede an AOT consumer's treatment to avoid Removals
