Leading product for mid-market and enterprise customers

Product for the Chronic Care Programs team at Carbon Health. We are responsible for building out patient-facing healthcare services for chronic conditions (e.g., diabetes, hypertension, and many more on the way) and the provider-facing tools that enable delivery of that care. These programs deepen Carbon's relationships with its primary and urgent care patients, delivering longitudinal care to patients who need it most.

I led the product for the Diabetes Program after Carbon's acquisition of Steady Health. We launched the first of its kind program that integrates diabetes specialists, primary care, and continuous glucose monitoring (CGM). With this launch, we also built the first and only EMR in the country that integrates CGM data and food and activity logging natively with all other patient records.More programs to enable patients to better manage chronic conditions in the works.

Steady was acquired by Carbon Health in May 2021.Joined as the first Product hire when Steady was seed stage. We were a team of 11, including 6 clinicians, building the best diabetes care experience on the market.
