Suicide Prevention/ Systems Error: Observed the effectiveness of the Safety Cell used to hold detainees who were at risk for suicide. Using the Healthcare Failure Modes and Effects Analysis (HFMEA) model, we found that the initial assessment questionnaire used to diagnose the level of risk for suicide was ineffective, therefore leading to misclassification of level of risk for suicide and ineffectively preventing suicide attempts in when held in the Safety Cell.Tuberculosis (TB) Reporting Process: Kept track of the TB reporting process to ensure that all detainees who were screened and diagnosed for active TB during initial intake process were on RIPE drug therapy.
