• Lead a team of DSW’s within residential house providing the highest standard of client focussed care to people living with disability.• The role is responsible for managing all aspects of site operation including rostering, staffing, recruiting, case management, maintenance, compliance, training, quality assurance, risk management, chemical orders, WH&S, coaching and guidance.

• Work with people to develop their own recovery plan that builds capacity, strength & resilience. • Collaborate with Support Coordinators, Allied Health team members, family, friends, support networks and other service providers to deliver best client care and outcomes.• Provide coaching and support to help people live their best life.

• The Wellbeing Facilitator is responsible for delivering high quality, customer driven supports in community settings.• Provide high quality services in line with the seven components of the NDIS Code of Conduct, and Candella’s guiding philosophy • Facilitate services in line with the customers goals, choices and preferences• Collaborate with a range of external stakeholders, including Support Coordinators, a customers’ family, friends or support network and other service providers to build relationships• Communicate any concerns, issues or complaints with the Candella management team

Provide support to people with disabilities including:• Community engagement• Domestic assistance• Learning support• Behaviour Management & Intervention• Coordinate after-hours enquiries, problems and rostering from both clients and staff .
