- Entered weekly referrals into FlexCare for mild to moderate impairments of MediCal-San Francisco Health Plan members who needed assistance with linkage to behavioral health services.- Called members to provide therapy and/or psychiatry referrals, and help with scheduling appointments to ensure that members are successfully connected to a mental health provider. - Created and submitted spreadsheets on Weekly Referral Tracker and Member Outreach Project to report on referrals’ status to SFHP Chief Medical Officer and Beacon Clinical Manager.- Collaborated with behavioral health providers, SFHP, and other Beacon colleagues to destigmatize mental health and advocate for members by ensuring that they receive accessible, affordable, and effective care to behavioral health services.
