The Headwaters Acquired Brain Injury Group (HABI) is run by and for people with acquired brain injury living in the Headwaters Region. HABI seeks to enhance lives for people living with ABI and their families through education, public awareness, support and social connection. We are member-driven focused on achieving common goals created by the group. We encourage you to be part of this innovative and pioneering approach to ABI support.• Shared responsibility in establishing and upholding group’s policies, procedures, and core values • Shared responsibility in assessing membership needs/wants with regard to creating group agenda and direction• Coordinate community ABI awareness events, educational workshops and social activities. (Brain Basics Training Program, ABI awareness BBQ, etc.) • Established strong community partnerships to promote ABI awareness, education and prevention • Co-facilitate monthly support group meetings

• Support follow-through and completion of rehabilitation programming developed by the team professionals, fostering independence and goal achievement • Consult with interdisciplinary team on a regular basis to ensure support is goal directed and outcome based• Promote and support the physical, psychosocial and spiritual well being of clients and families• Facilitate community reintegration, connecting individuals to appropriate community-based resources• Support individuals in establishing a normative daily routine

• Manage caseload of 15-20 client files• Supervise RSW/PSWs to ensure client-centred support is offered while supporting the client’s and team professional’s directives• Manage quality of service through RSW support, education and consultation with client, family and rehabilitation team • Manage administrative duties pertaining to client file, including HCAI responsibilities, editing progress reports and scheduling matters• Attend ongoing training and relevant conferences to develop ABI knowledge• Foster positive working relationships with team professionals and colleagues in the ABI community
