Integrate the quality management / performance improvement program with clinical and business goals and objectives. Participate in gathering, trending, and reporting data related to the oversight, planning, and coordination of Quality management activities, including patient care reviews, compliance audits, concurrent and retrospective clinical record reviews for the interdisciplinary team. Complete research, analysis, and report preparation based on audits and case reviews to make recommendations to leadership to ensure documentation quality. Field and respond to complaints and inquiries, interview and confer with relevant parties, document all interactions, and work with leadership on resolving escalating situations.

Direct and oversee daily operations for a national division responsible for providing seamless transitions into hospice care by way of comprehensive patient education and care coordination. Provide education for internal and external group(s) of one-to-one instruction, up to company-wide; via in-person, phone conference, and/or webinar format to ensure extensive understanding of the unique challenges with this population. Develop and implement effective processes aligned with company goals for growth and clinical excellence. Provide clinical oversight to direct report nursing staff regarding patient eligibility and regulatory processes while focusing on medically complex situations. Serve as a mentor for hospice operations regarding complicated case scenarios and compliant recommendations for hospice admission criteria with supporting documentation.

Develop, implement, and review educational items specific to identified needs for 25 plus agencies within the region. Lead contact for clinical compliance and regulatory inquiries from the agency level. Regulatory guidance consultant to the local agency before and during compliance surveys. Provide continued support to ensure an effective plan of correction that meets the needs identified in the survey, if any. Point of contact for agency guidance related to the Accreditation Commission for Health Care (ACHC) survey. Deliver education to group(s) of one-to-one instruction, up to company-wide; via in-person, phone conference, and/or webinar format to ensure agency staff receives key and up-to-date information regarding company policy and regulatory practices. Collect local agency documentation for the region to analyze data outcomes and utilize company-wide for quality improvements. Develop education based on negative trends identified to deliver to agency Quality Assurance / Performance Improvement (QAPI) coordinator. Contributing member of the organization policy committee. Review, revise, and create clinical and organizational policies to implement at the agency level. Chair of organization forms committee. Review, revise, and create paper and electronic forms required to comply with company policy and regulatory requirements.

Responsible for the day-to-day operations of assigned Hospice program/site. Plans, organizes, directs and evaluates services of Hospice at assigned location. Directs and coordinates admission of patients to Hospice Services in a smooth, systematic process, as well as arranging for ancillary services so that they are complimentary and non-duplicative. Directs, supervises and evaluates hospice personnel and volunteers and coordinates services of all team members of the multi-teamed site.
