Band 7 Specialist Dietitian working within the Sutton Carehome Support multidisciplinary team. We support 27 care homes in Sutton Borough providing end of life care and a Liason service. My role involves dietetic assessments, education and training to help care homes meet the nutrition and hydration needs of their residents as required by the CQC. I also work closely with locality teams in the Sutton region including the medicines optimisation team and pharmacists to promote appropriate prescribing of ONS supplements providing evidence of savings to the NHS which is achieved through audit & research, training and dietetic interventions. In turn, such interventions holistically contribute improved health outcomes in the elderly.In addition, I am leading on the Southwest London care home Nutrition & Hydration strategy in line with the SWL EHCH transformation programme and working with Colleagues and key stake holders across the CCG.
