 Provide comprehensive care management services to a caseload of 50 -100 clients with multiple chronic conditions and severe substance abuse or behavioral health issues  Conduct an Assessment within 30 days to understand the medical, behavioral and social needs of each person. - Provide people living with HIV/AIDS with counseling and case management services, decreasing barriers and increasing benefits to promote medication and treatment adherence.- Conduct individual counseling sessions with 20-25 clients to assist them in coping with the emotional stressors of living with HIV/AIDS. Address issues of stigma, guilt, harm reduction around substance abuse and high risk sexual behaviors, body image issues, barriers to adherence, immigration issues, family matters, housing issues, mental health issues, LGBTQ issues, homelessness and disclosure around HIV and Sexual Orientation.- Conduct initial intake of client and periodic reassessments. Screen for suicide and homicidal ideation, domestic violence, substance abuse and other risky behaviors.- Create service plans with client to outline short-term and long-term goals and to identify barriers in reaching goals. Complete documentation of daily services and create/ maintain client files. Enter billing in AIRS program.- Coordinate care with other service providers to assist client in reaching goals outlined in service plan.- Provide group counseling for over 30 clients by facilitating and co-facilitating five different support groups a week with different populations all living with HIV/AIDS. Topics discussed in groups included, but not limited to, treatment adherence, healthy living, current events, risk reduction around substance abuse and sexual risky behaviors, stress management, benefits, and traumas.
