Long Term Care Pharmacist
Current- Provide patient-centered care and medication services for patients residing in hospice, assisted-living, and skilled-nursing facilities- Review medication orders to ensure appropriate dosing, route of administration, and monitor for any allergies, drug-drug and drug-disease state interactions- Lead, manage, and work with pharmacy technicians to ensure the safe dispensing of medications (i.e., IV medications are compounded correctly and under sterile conditions), and to navigate any third party issues that may arise- Work inter-professionally with other health care providers (i.e., physicians, nurses) by answering medication-based questions that require the expertise of the pharmacist- Ensure that any potential delays in processing medication orders with respect to cost approvals, prior authorizations, and safety concerns are promptly shared with the appropriate LTC party/representative- Monitor and adjust dosing regimens for patients on intravenous vancomycin based on parameters such as drug concentration levels, renal function, and complete progress notes for skilled nursing facilities- Supervise medication management systems to ensure that adequate supplies of medications are available to meet resident care needs, stored under appropriate and secure conditions, delivered to residents in a timely, safe, and efficient manner, and labeled appropriately- Oversee proper inventory management and medication ordering- Assist with and oversee medication returns and E-Kit management- Be on-call to ensure any urgent medication orders are received, processed and delivered in a timely manner and/or answer provider questions
