Facilitates inter disciplinary team meetings, reviews outcomes with person and case manager and other team members quarterly, and maintains communication through oral and written reports.• Assists new and transitioning participants with moves in the community and assures immediate needs are met.• Maintains records for each person including the relevant data as specified in Consolidated Standards.• Continue ongoing educational training as to render quality services and gain knowledge of new ideas and concepts in the field.

6 week clinical rotation where time was spent providing physical therapy treatment as deemed by the physical therapist and physical therapists assistant in a transitional care facility. Including but not limited to therapeutic exercise, therapeutic activity, neuromuscular re education, modalities and gait training therapy.

As a SPTA providing treatment for spine, sports and occupational injuries, and all orthopedic and neurological disorders for patients of all ages.  Promoted healing with a combination of hands on manual therapy, pain relieving modalities, and exercise.  Also instructing patients in injury prevention, body mechanics and self-help techniques.

Providing physical therapy in a transitional care, outpatient, skilled nursing and home health care facility. Carrying out specific plan of care that provides appropriate care for patients. Also accurate documentation as required by facility policies.

In charge of managing staff. This includes testing job applicants, as well as providing daily reports to supervisors. Implementing programs to persons served within assisted living settings.
