With ‘collaboratehealth’, patient care is enhanced and extended beyond the traditional in-person doctor/patient visit at hospitals and clinics. This is achieved while maintaining a high level of quality care, unquestionably, a shared requirement for both the patient and medical team.We appreciate the workload of medical teams, and with ‘collaboratehealth’, have created an intuitive experience that offloads common tasks and procedures, freeing precious time to focus on vital patient issues. Upon discharge, customized patient care plans are automatically generated to ensure a successful transition back home. Bedside care with web-side follow-up provides an important patient/doctor connection.Our primary focus is for patients with critical, long-term support needs. Our data confirms that ‘collaboratehealth’ improves the discharge success rate and subsequent follow-up care for patients in all hospitals that we currently serve. It’s very gratifying for us to know that with ‘collaboratehealth’, patients are able to continue their healing with timely and specific medical support, all from the comfort and convenience of their own home, to ultimately feel their very best once again. Equally important is that with ‘collaboratehealth’, we’re able to scale and offload the medical team by 1) providing an intuitive and convenient method to follow-up with patients, and by 2) automating common tasks. This allows the medical team to focus on what matters most, the best use of their precious time, and, the critical and timely care of patients.
