Service Navigator
Current• Provide intake and referral support for community agencies, by connecting families to various clinical and community supports, including Occupational Therapy, Physiotherapy, Developmental Assessment Consultation Services, Feeding and Assessment Consultation Services, and psychological services, and the Ontario Autism Program Entry to School, Caregiver Mediated Early Years, and Urgent Response Services. • Significant experience in working with multiple organizations to provide ongoing integrated care, and service planning. • Strong administrative skillset, including organizational data entry, ongoing case management and phone and email communication, to provide families with informational resources about medical clinics.• Able to focus on detailed tasks to efficiently complete research, documentation, report writing, work instructions, data management, and customer service.• Take initiative to collect, evaluate, and utilize client information to develop a coordinated service plan to meet identified needs, while exercising sound judgement and adaptability.• Maintain confidentiality, discretion, and accountability, while professionally interacting with clients, families, staff, community service providers, external medical offices, school boards, and York and Simcoe region providers.• Commitment to creating a safe and inclusive space for staff, clients, and families, through understanding various cultures, and remaining in compliance with the Occupational Health and Safety Act as a certified Joint Health and Safety Committee (JHSC) member.
