HARP is a community program based in tertiary hospitals that manages self-management education and best practice support of clients with chronic disease.Responsibilities· Leadership and support for a multidisciplinary team supporting chronic and complex clients.· Management of referrals and streamlining care processes.· Clinical governance and quality improvement initiatives.· Management support and education of staff including performance management and change management.Achievements· Quality improvement project using LEAN for pulmonary rehab for COPD clients at Northern Health.· Change management in the implementation of comprehensive assessment form based on NHQHS standards.· Performance management of underperforming staff putting patient care at risk.

A home visit-based Advance Care planning service for end stage COPD clients admitted to the HARP Respiratory program at Northern Health.Responsibilities· Advance care planning and education for palliative care clients.· General practitioner engagement and end-of-life symptom management.· Collaboration with multidisciplinary teams.Achievements· An increase in the acceptance, uptake and completion of advance care plans for COPD clients within the Northern health

Darebin Community health was the first Community health centre to incorporate a HARP program within the community setting.
