CurrentResponsible for all aspects of coordination of patient evaluation for advanced heart failure therapies from referral through listing for transplant or ventricular assist device placement while maintaining effective use of hospital and clinical resources.Provide initial education and continually assesses need for ongoing education to patient and families through the transplant and MCS care continuum.Closely monitor patients with NYHA Class III / IV and AHA/ACC Stage C/D heart failure status while being evaluated for advanced heart failure therapies. Responsible for monitoring all patients listed with UNOS while awaiting heart donors.Participate in the development and implementation of protocol and policies for the pre-transplant patient.Provide on call coverage for all solid organ transplantation to include coordination of activities prior to transplant, communication between physicians, local organ procurement organization, histocompatibility lab, blood bank, patient / families and medical team involved.Perform all necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines to include listing, status changes and post-transplant follow up forms.Coordination of inpatient mechanical circulatory support patient care including intra-operative device management and participation in post-operative preparation for discharge.Coordination of long term management and follow up care for outpatients post VAD implant.Provided on call coverage for the mechanical support device program, supporting patients in need of emergency advice for the VADs. Provided education to nursing staff ongoing basis for mechanical support devices.Facilitate ongoing data collection for INTERMACS, the national data registry for mechanical circulatory support.
